Support Coordination

What is support coordination? 

Support coordination is a specific type of support which can be funded in a participant plan. Support coordinators help participants understand and utilise the funding included in an NDIS plan. Support coordinators can also assist with connections with other community and mainstream services. 

What does support coordination include?

Here is a list of things a support coordinator may help with:
  • Help you understand your NDIS plan - Support coordinators will help you understand the types of support available to you, how you can use your funding and what other requirements are needed to access these supports (including quotes/claims and bookings). 
  • Plan and coordinate your support services - Support coordinators will help find service providers and supports that are needed to meet your goals. Coordinators will ensure the services are tailored around participants needs & availability. 
  • Connect you with supports and services - Support coordinators will help find local support/service providers. 
  • Establish and maintain supports - Support coordinators will help manage service agreements to make sure NDIS participants get the most for what they are paying. 
  • Coach refine and reflect - Support coordinators can help NDIS participants build confidence and skills to help utilise a plan. 
  • Report to the NDIA - Support coordinators help manage all of the reporting requirements for NDIS participants including: initial implementation reports, mid-term implementations reports and plan reassessment reports. 
  • Capacity building for independence - Support coordinators enable NDIS participants to exercise choice and control over their plans. Coordinators will help with communicating the types of supports required for participants to meet their goals. 
  • Prepare for unexpected situations - Support coordinators can help manage and work through unexpected problems and complexity in an NDIS participants plan. 
  • Acting in your best interests - Support coordinators act in your best interests by prioritising your goals.
